Instruction on the processing of personal data and consent to the processing of personal data

I hereby confirm that I have been informed about the processing of personal data and, as a data subject, I grant express, unreserved and voluntary consent to the processing of personal data to the extent specified below, for the purpose specified below and for the period specified below, in accordance with § 13 of the Act no. 18/2018 Coll. on the protection of personal data and on the amendment and addition of certain data as amended (hereinafter referred to as "Act No. 18/2018 Coll."), Regulation of the European Parliament and Council (EU) No. 2016/679 of April 26, 2016 on the protection of natural persons in the processing of personal data and on the free movement of such data, which repeals Directive 95/46/EC (General Data Protection Regulation), as well as other relevant legal regulations.

 

The controller of personal data is the civic association mMEDCON oz, with headquarters at Mostište 40, 020 51 Dohňany, ID: 36 115 908, e-mail: mmedcon@gmail.com (hereinafter referred to as the "Controller").

 

The person responsible for the processing of personal data is: Anna Malová, e-mail: nnmalova.na@gmail.com

 

The Operator processes personal data for the purpose of ensuring participation in the Correspondet Seminar in Medicine and evaluating the work of participants (hereinafter referred to as the "Program"), as well as for the purpose of ensuring all related events, for the purpose of cooperation with Program partners, for the purpose of promoting the Operator, partners Program and the Program and for the purpose of securing all related and necessary legal and factual actions. 

 

The person concerned hereby further agrees to the Operator publishing processed personal data for the purpose of promoting the Operator, Program partners and the Program in the scope of: name, surname, e-mail address, solutions to assignments from the Program, points obtained for assignments from the Program, photos from the Program , audiovisual recordings from the Program, prepared outputs from the Program, opinions and reviews of participants, profiles of participants, or other data absolutely necessary for promotion.

The affected person grants consent to the processing and storage of personal data for the period necessary to ensure the affected person's participation in the Program and ensure the promotion of the Operator and the Program.

The affected person has the right to request from the Operator access to personal data concerning the affected person (the right to information regarding the scope and purpose of processing), the right to correct personal data, the right to delete personal data, the right to limit the processing of personal data, the right to object to the processing of personal data , as well as the right to the accuracy of personal data. The person concerned can withdraw his consent at any time and exercise the above-mentioned rights, especially by sending to the e-mail address mmedcon@gmail.com. The affected person also has the right to file a motion to initiate proceedings pursuant to § 100 of Act no. 18/2018 Coll. as well as other rights arising from Act no. 18/2018 Coll. and other generally binding legal regulations.

The affected person agrees that the Operator will provide the personal data of the affected person to other persons participating in the implementation of the Program, for the purposes of cooperation with the partners of the Program and promotions of the Operator and the Program.
